This is the one page that turns "congratulations, you graduated" into an actual standing connection — collected the night a student crosses the finish line, before anyone has a chance to lose touch. It captures contact info, parent or guardian details if the graduate is still a minor, a one-sentence read on whether their business is still running, and three real choices: opt in to the quarterly check-in, raise a hand to come back as a future coach or mentor, and decide whether their story can be used in WCEA's press, social media, or grant materials. A staff-only section at the bottom confirms it's actually been entered into the alumni tracker — so a signature on event night doesn't quietly disappear into a folder.
