This is the form that closes the gap between WCEA's general liability insurance and what a specific business actually needs covered — required whenever a student's idea falls into one of nine categories: food requiring refrigeration, cosmetics, candles, children's products, animal handling, physical activity services, short-term rentals, personal care services, or transportation and delivery. It walks each category through what's genuinely different about it, makes clear the family bears responsibility for confirming the actual permits, licensing, or safety standards that apply, and requires sign-off before that business can sell at the Soft Launch Event, Pitch Night, or Demo Day. It closes with the point that matters most: this isn't a denial — it's how no family gets surprised later by something they could have known now.
